Booking an eye examination

 If you require an eye examination, please be aware that due to high demand we are having to prioritise those patients who need to be seen first, therefore there may be a delay in arranging an appointment. We are asking patients requesting an eye examination to complete a quick pre-exam questionnaire so that we may arrange an appointment within a suitable time scale.
